Health happens where we live, work, and play; not only within the walls of the hospital or clinic. However, for many of our most vulnerable populations, access to services and support, doctors and hospitals, is limited. For the vitality of our community, we must meet our patients where they are, help them get well, and live well. And, we must maintain these connections consistently, especially during the most challenging of times like the current COVID-19 pandemic. During this session we will discuss the unseen barriers to closing the last mile to neighborhood health services and how technology can help lower these barriers to create healthy and happy communities.
